Why do some videos produce no frames?

The browser can only decode codecs it supports. HEVC (H.265) cannot be decoded in most browsers — the video may play while every captured frame comes back black. Convert to H.264 mp4 first.

Why is extraction capped at 60 frames?

Each frame needs a serial seek, which is slow. A two-hour video at a one-second interval is over 7,000 frames and would hang the tab. Split the video or increase the interval when you need more.
